Prescription Program
Effective July 1, CVS Caremark replaced Express Scripts as the Benefit Funds’ pharmacy benefit provider. Due to this change, please note the following:
- Submit acute and maintenance drug prescriptions in The 90 Day Rx Solution to CVS Caremark Mail Service Pharmacy for mail order fulfillment; if your 1199SEIU patient prefers to pick up the medication at a retail pharmacy, submit prescriptions to a CVS, Rite Aid, Walgreens or Duane Reade pharmacy;
- Submit specialty drug prescriptions to CVS Caremark instead of Accredo; and
- Review the CVS Standard Control with the Advanced Control Specialty Formulary for formulary information on the Benefit Funds’ website or call (833) 250-3273.

Reimbursement for Drugs Purchased on or Before June 30, 2024
Use the Express Scripts Direct Claim Reimbursement Form for drugs purchased on or before June 30, 2024. For purchases on or after July 1, 2024, use the related CVS Caremark form above.
