We are recredentialing our Provider Network and we need your help. Please update your Council for Affordable Quality Healthcare (CAQH) application and your attestation form. You also must re-attest to the accuracy of the information in your profile, including the address(es) for your practice.
Please visit proview.caqh.org to update your information, including your:
- Current malpractice policy face sheet;
- W-9 for each new billing location; and
- Complete five-year work history.

Additionally, we have partnered with CredSimple, an NCQA-certified Credentials Verification Organization, to obtain any missing information and validate the demographic information on your CAQH application, so please remember to grant us access to your profile.
